Patient survey about virtual care for psoriasis and psoriatic arthritis

Have you used virtual care in your treatment journey?  
 
The Canadian Association of Psoriasis Patients, the Canadian Psoriasis Network, and the Canadian Skin Patient Alliance are collaborating on a project to gather patient perspectives on integrating teledermatology into psoriatic disease care. 
 
We want to hear from you about your experiences with telephone, online, or virtual appointments with your healthcare provider.   This survey takes 5-8 minutes to complete and will give us valuable insights into the patient perspective on how telemedicine can improve psoriasis and psoriatic arthritis care in Canada.
